Skill Training Information Sheet and Payment Contract

All skill sessions are individualized, meaning the
athlete to coach ratio is 1:1. Each client has the
option of having more athletes at skill sessions if
desired.
• Hoop Smarts, LLC will assess a one-time, non
refundable $50 registration fee. At the time of registration,
Hoop Smarts will connect each client with a skills
coach.
• Each client must choose the number of sessions
they wish to have per month. The options are: 4 (one
session per week), or 8 (two sessions per week).
Prices for skill sessions are:
4 sessions per month- $150
8 sessions per month- $300
• Charges will be invoiced on a monthly basis.
The client will receive an invoice for the month at
the beginning of the month. All payment is due in
advance.
• After Hoop Smarts connects a client and coach,
and the client had chosen the number of sessions,
it is to be decided between the client and coach when
the sessions will occur. If a session is missed for
any reason, the client and coach are to arrange a
time for a make-up session. Refunds will not be issued
for cancellations.
• All payments must be received by the first
session of each month. If payment is not received,
sessions will be suspended until payment is rendered.
